Adenomyosis (pronounced A - den - oh - my - oh - sis) is a medical condition characterized by the presence of ectopic glandular tissue found in muscle.[1] The term adenomyosis is derived from the Greek terms adeno- (meaning gland), myo- (meaning muscle), and -osis(meaning condition). Previously named as endometriosis interna, adenomyosis actually differs from endometriosis and these two disease entities are found together in only 10% of the cases.[2]
It usually refers to ectopic endometrial tissue (the inner lining of the uterus) within themyometrium (the thick, muscular layer of the uterus). The term "adenomyometritis" specifically implies involvement of the uterus.[3][4]
The condition is typically found in women between the ages of 35 and 50. Patients with adenomyosis can have painful and/or profuse menses (dysmenorrhea & menorrhagia, respectively). However, because the endometrial glands can be trapped in the myometrium, it is possible to have increased pain without increased blood. (This can be used to distinguish adenomyosis from endometrial hyperplasia; in the latter condition, increased bleeding is more common.)
In adenomyosis, basal endometrium penetrates into hyperplastic myometrial fibers. Therefore, unlike functional layer, basal layer does not undergo typical cyclic changes with menstrual cycle.[2]
Adenomyosis may involve the uterus focally, creating an adenomyoma. With diffuse involvement, the uterus becomes bulky and heavier.

Signs and symptoms

Some women with adenomyosis do not experience any symptoms, while others may have severe, debilitating symptoms. The endometrial implants that grow into the wall of the uterus bleed during menstruation (the same as endometrial tissue bleeds) and are discharged vaginally as menstrual bleeding. The vaginal pressure can be severe enough to feel like the uterus is trying to push out through the vagina, like the first stage of labor when the baby's head pushes into the cervix. Other symptoms include;
  • Intense debilitating pain all the time and/or
  • Acute & increasing pain at menstruation and ovulation
  • Strong 'contraction' feel of uterus
  • Abdominal cramps
  • A 'bearing' down feeling
  • Pressure on bladder
  • Dragging sensation down thighs and legs
  • Heavy bleeding and flooding
  • Large blood clots
  • Prolonged bleeding i.e.; up to 8–14 days

Complications

Adenomyosis is associated with an increased incidence of preterm labour and premature rupture of membranes.[5] Women with adenomyosis are at an increased risk of anemia. This can cause fatigue, dizziness, and moodiness. Adenomyosis has also been linked with anxiety, depression, and irritability.
review in 2012 found no evidence that adenomyosis should be detected and treated in patients who seek assisted reproduction.[5]

Causes

The cause of adenomyosis is unknown, although it has been associated with any sort of uterine trauma that may break the barrier between the endometrium and myometrium, such as a caesarean sectiontubal ligationpregnancy termination, and any pregnancy. It can be linked with endometriosis,[6] but studies looking into similarities and differences between these two conditions have conflicting results.[7]
Some say that the reason adenomyosis is common in women between the ages of 35 and 50 is because it is between these ages that women have an excess of estrogen (Estrogen Dominance) [citation needed]. Near the age of 35, women typically cease to create as much natural progesterone, which counters the effects of estrogen. After the age of 50, due to menopause, women do not create as muchestrogen.
Adenomyosis correlates with abnormal amounts of multiple substances, possibly indicating a causative link in its pathogenesis, althoughcorrelation does not imply causation:

Diagnosis

The uterus may be imaged using ultrasound (US) or magnetic resonance imaging (MRI). Transvaginal ultrasound is the most cost effective and most available. Either modality may show an enlarged uterus. On ultrasound, the uterus will have a heterogeneous texture, without the focal well-defined masses that characterize uterine fibroids.

MRI provides better diagnostic capability due to the increased soft tissue differentiation, allowable through higher spatial and contrast resolution. MRI is limited by other factors, but not by calcified uterine fibroids (as is ultrasound). In particular, MRI is better able to differentiate adenomyosis from multiple small uterine fibroids. The uterus will have a thickened junctional zone with diminished signal on both T1 and T2 weighted sequences due to susceptibility effects of iron deposition due to chronic microhemorrhage. A thickness of the junctional zone greater than 10 to 12 mm (depending on who you read) is diagnostic of adenomyosis (<8 mm is normal). Interspersed within the thickened, hypointense signal of the junctional zone, one will often see foci of hyperintensity (brightness) on the T2 weighted scans representing small cystically dilatated glands or more acute sites of microhemorrhage.
MRI can be used to classify adenomyosis based on the depth of penetration of the ectopic endometrium into the myometrium.
Exact diagnosis of adenomyosis only possible in posthysterectomy specimen.[2]

Treatment

Conservative treatment often consists of anti-inflammatory medications, such as ibuprofen or other NSAIDs. Hormonal manipulation may include combined or progestin-only oral contraceptives as well as other options such as transdermal patches and progesterone-releasing intrauterine devices. Hormonal suppression may be used in the form of danazol or a gonadotropin-releasing hormone agonist. These medications simulate menopause, a period in which adenomyosis often resolves naturally.
Based on one's desire for future fertility and the size of the lesion, surgical options may include endometrial ablation, laparoscopic myometrial electrocoagulation and adenomyoma excision. These have demonstrated positive results in several studies, though long-term data is lacking. A non-surgical procedure, uterine artery embolization may also be used to block the blood supply to the ectopic endometrium, selectively killing the problematic tissue. Hysterectomy may be warranted in some cases where fertility is not desired, and is the only definitive treatment for the disease.[10]

Prognosis

It is advocated that adenomyosis poses no increased risk for cancer development. However, both entities could coexist and the endometrial tissue within the myometrium could harbor endometrial adenocarcinoma, with potentially deep myometrial invasion.[11] As the condition is estrogen-dependent, menopause presents a natural cure. Patients with adenomyosis often also have leiomyomata and/orendometriosis.

  1. This information will help those suffering from Adenomyosis and Endometriosis, I was also a victim. I am excited to share this testimony, I was 3 years ago diagnosed of Adenomyosis and complicated Endometriosis. My medical condition was heart broken because it was causing my menstrual cramps,bloating before menstrual periods, lower abdominal pressure which result to heavy periods. I was subjected to different medications by my doctor for treatment such as Coenzyme Q10,ibuprofen, Danazol and many more. Despite my visit to several doctors my health wasn't getting better, all they could say was surgery. At the verge of giving up, I went to the internet to search for a treatment at least.     I suffered from severe debilitating pelvic pain for over 9 years. It was thought that I had endometriosis but I had really adenomyosis. It was diagnosed after my hysterectomy. I also had multiple fibroids.

  3. I have large uterine fibroid and multiple fibroid all over my womb (womb measures 15cm). I had a uterine fibroid embolisation in April last year. Before this treatment my womb measured 23cm. I'd like to share my success story with natural remedy: I had advanced adenomyosis was causing me to have the most excruciating pains every month and very heavy bleeding and of course a chronic anemia. The first gynae I saw said the only option was to remove my uterus. At only 24 my left ovary and Fallopian tube was removed due to endometriosis. After 4 years I had adenomyosis and right fallopian tube blocked. Always wanted to conceive but never been pregnant because adenomyosis was grown that I never had dimensions. My period came and was very heavy with clots - lasted 5 days and stop for 2 - came back again medium heavy for another week. I started taking Tranexamic acid for 5 days, Nature's way DIM-plus, four (4) capsules once a day. I also took the Pure Life Essence for Women, also 4 capsules once a day. I mix apple cider vinegar and water and take it twice a day and didn't work, it minimizes flow but comes back again, my Doctor gave me progesterone 200 mg and I kept bleeding...it's took me 3 weeks of bleeding; I looked very pale and felt weak - missed a couple of days at work.   6. I have suffered from adenomyosis with severe cramps during and after my period. It was very painful and heavy bleeding and severe lower back pain my pain was unbearable to a point of being not able to move and a stabbing like pain on the left of my ovaries. Pain goes away when a clot comes out. But it took a while for the clot to come out. After several meetings with my gynaecologist which she suggested "Hysterectomy" but I refused I know what Hysterectomy is and how heart aching it can be then she stated that Allopathic treatment will help in pain management but it will not cure the disease. I started treatment with a drug named "endoheal 2 mg". This gave me pain relief but reduced my periods almost to no periods with numerous side effects - spotting and fluctuation in my dates. I was not mentally satisfied to bear the side effects so I came across ''Ayurvedic doctor" who started my treatment with herbs. Though I continued above drug 'endoheal 2 mg'" parallel for nine months for easy pain management. From then I continued taking the treatment. It reduced the size of my lesion but did not reduce it further after few months. And there was no relief in pain during menstruation but my Dr. Advised me to continue it during 3 months without any gap. I had noticed a quick ageing of my skin in the last 2 years. I am a smoker but for some reason I feel my quick ageing has to do with something else because I have been smoking for a long time and it's only recently that I noticed a fast decline of my skin elasticity. After then pain radiates very badly in my left leg, lower left back and left side of my vagina. The pain normally start anytime during 3, 5,6 days, but at time I had to take a painkiller for this also as it irritates whole day. Then pain disappears after fews day.
